Three forms with handwritten responses and photos. A note is attached from the Collector of Int. Rev, District of Oregon saying that Chin Wing Fook submitted the same facts on Janurary 10, 1894 in Portland Oregon. All forms are signed by Chin Wing Fook. The last two forms are signed by the Deputy Collector of Customs, and the Chinese Inspector. Also attached are some handwritten notes on the back of a check from the Boston National Bank.
