The form is dated May 8, 1953 in the upper left corner and has "Mr. Green" typed in the upper right corner. The upper right corner also has a fold. 1992.064.069B is the accompanying envelope, measuring 4 1/8" x 9 1/2," written in the upper right corner of the front, in pencil, is "Shipping Venesuela." The lower right corner is bent. The envelope is a dark manilla color.
